    Your Path to Sterilization: Step by Step

    1. Personal Consultation

    In an in-depth initial consultation with Dr. Johannes Seidel or Dr. Jens Kumposcht, we discuss:

    • The different sterilization methods
    • Your individual wishes and motivations
    • Possible risks and alternatives

    👉 Additionally, a gynecological examination with a detailed vaginal ultrasound is usually performed to assess the uterus, ovaries, and abdominal cavity, and to detect any pathologies early.

    2. Preparation for the Procedure

    After your decision for sterilization, we promptly organize all necessary pre-examinations and surgery appointments.

    You will receive detailed information about the procedure, preparation, and aftercare.

    3. The Procedure

    The minimally invasive procedure is performed on an outpatient basis under anesthesia. We use the laparoscopic method, which is distinguished by its benefits:

    • Duration: Approximately 45 minutes
    • Technique: A camera is inserted through a small incision at the navel. The fallopian tubes are removed through 1–2 additional tiny incisions (approx. 5 mm) in the lower abdomen.
    • Anesthesia: General anesthesia
    • Setting: Day surgery at Klosterneuburg Hospital or Rudolfinerhaus Private Clinic
    • Discharge: Admission in the morning – discharge usually in the afternoon

    4. Aftercare and Recovery

    Recovery is usually quick:

    • Healing: Small incisions, self-dissolving sutures, healing within a few weeks
    • Discomfort: Temporary mild lower abdominal pain or shoulder discomfort due to the CO₂ gas used in laparoscopy
    • Daily Life: Socially functional usually from the next day; return to work after about 2 days
    • Sports, Swimming, Intercourse: Recommended after about a 1-week break
    • Long-term: Cycle, libido, and hormone production remain unchanged

    Methods

    Choosing the right method is crucial. At Woman & Health, we prefer laparoscopic bilateral salpingectomy, as it is the safest and most modern method. Here’s an overview:

    Laparoscopic Bilateral Salpingectomy

    Both fallopian tubes are completely removed. This is the current medical standard and our preferred method.

    Advantages

    • Maximum Safety: Failure rate close to 0% – pregnancy is virtually impossible.
    • Additional Protection: Significant reduction in the risk of ovarian, tubal, and peritoneal carcinomas.
    • No Hormonal Changes: Cycle, libido, and hormone production remain unchanged.

    Disadvantage

    • Not reversible. Pregnancy is only possible through in-vitro fertilization (IVF).

    Duration

    • Approximately 45 minutes, minimally invasive (laparoscopic).

    Suitable for

    • Women who want absolute certainty and no longer wish to have children.
    • Women with a family history of increased ovarian cancer risk.

    Laparoscopic Sterilization with Tubal Clips

    The fallopian tubes are closed with special clips, preventing sperm from reaching the egg. This method is now only performed in exceptional cases due to its higher failure rate.

    Advantages

    • Potentially reversible (re-fertilization possible; success rate highly dependent on the surgeon, approximately 30–60%).
    • Very short, minimally invasive procedure.

    Disadvantages

    • Significantly Higher Failure Rate compared to salpingectomy (complete removal of both fallopian tubes): Pearl Index 0.5–1 (approx. 1–2 pregnancies per 100 women).
    • Increased risk of ectopic pregnancies.
    • No additional benefit in reducing ovarian cancer risk.

    Suitable for

    • Women who want sterilization but wish to keep the option of re-fertilization open.
    • Note: Even after successful re-fertilization, there is an increased risk of ectopic pregnancies.

    Laparoscopic Coagulation and Cutting of a Tubal Segment

    The fallopian tubes are coagulated and a segment is cut.

    Advantages

    • Lower failure rate than tubal clips: Pearl Index approximately 0.2–0.5 pregnancies per 100 women-years.
    • Minimally invasive, short procedure.
    • Theoretically reversible.

    Disadvantages

    • Higher risk of ectopic pregnancies in case of failure.
    • Limited reversibility (approximately 20–30% success rate for re-fertilization surgery, with a significantly increased risk of ectopic pregnancies afterward).
    • No reduction in ovarian or tubal cancer risk.

    Suitable for

    • Women who want a safe method but wish to keep a small option for reversibility.
    • Women with previous surgeries or adhesions, where complete fallopian tube removal may be technically more difficult or risky.

    Postpartum Tubal Sterilization

    The fallopian tubes are ligated and a segment of approximately 0.5 cm is removed immediately after childbirth. This can be done in the first days after a vaginal delivery (usually via a small incision at the navel) or during a cesarean section. Complete removal of both fallopian tubes is also possible during a cesarean section.

    Advantages

    • Can be performed without an additional procedure if a cesarean section is already taking place.
    • Minimally invasive (via navel incision after vaginal delivery or intraoperatively during cesarean section).

    Disadvantages

    • Only possible immediately after childbirth.
    • Depends on the method (ligation or complete removal).

    Suitable for

    Women who want definitive sterilization immediately after childbirth.

    Why Complete Fallopian Tube Removal?

    Complete fallopian tube removal is now the international medical standard.

    • It is the only sterilization method with nearly 100% contraceptive reliability.
    • Many precursors to ovarian cancer (STIC – serous tubal intraepithelial carcinoma) originate not in the ovary but in the cells of the fallopian tube lining. Removing the fallopian tubes significantly reduces the risk of developing ovarian cancer later in life. Therefore, we recommend discussing fallopian tube removal with any planned gynecological surgery in the abdominal cavity when family planning is complete.
    • The procedure has no impact on the hormonal production of the ovaries and, when performed correctly, does not lead to premature menopause.

    Important Note: With all methods where the fallopian tubes are not completely removed, there is a (rare) risk of recanalization, which could lead to an ectopic pregnancy.

    International patients The total cost for sterilization for patients without Austrian social insurance (international patients) is approximately EUR 3,200–3,300.
    InsurancePrivate health insurances generally do NOT cover the costs of sterilization, possibly for additional necessary procedures. In exceptional cases with an increased risk of ovarian cancer, reimbursement may be possible. Prophylactic salpingectomy to reduce the risk of ovarian cancer is standard when family planning is complete and may sometimes be covered by health insurance.
